This is to inform the general public that the State School of Midwifery, Sapele Admissions Forms is on Sales.
Accordingly, the Common Entrance Examination Committee (Nursing and Midwifery) announces its Entrance Examination into the following Schools of Midwifery in Delta State for admission of Students into the 18 Calendar month Post Basic Midwifery Programme in September 2015 for the award of the Registered Midwife (R.M) Certificate.
(i) State School of Midwifery, Sapele
ENTRY QUALIFICATION:-
(i) Five Credits at SSCE/WASC O/Level at one or two sittings including English Language, Biology, Chemistry, Physics and Mathematics
(i) Candidates must be Registered Nurses (R.N)
APPLICATION FORM:-
Candidates are requested to apply for forms, with a non-refundable Entrance Examination fee of N6,000.00 and 2 passport sized photographs to the Principal of applicant’s School of choice.
ENTRANCE EXAMINATION DATE AND VENUE:-
DATE: Wednesday, August 5th 2015
TIME: 10 a.m. prompt
VENUE: State School of Midwifery, Asaba
N/B: CANDIDATES SHOULD ENDEAVOUR TO BRING THE FOLLOWING ALONG WITH THEM ON THE DAY OF THE EXAMINATION.

(iii) Notification of Registration (R.N.)
